Students applying for Annual Vacation must meet the following conditions: 1) Have been enrolled for 3 consecutive quarters for full-time studies, and maintained F-1 full-time student status. 2) Have zero balance at Washington University of Science and Technology or written consent from the financial office permitting leave. 3) Intend to register for the quarter following the leave.

Please remember F1 students can only take one quarter off at a time.
Please only use this form if you are an F-1 student and there is an emergency that requires you to travel outside the United States while classes are in session. Please remember to only use this form is you are requesting a short emergency leave, not a leave of absence.
