# Payment Plan fee is $30 (Non Refundable).
# Each late payment will result in a late fee of $50.
# It is the student’s responsibility to meet their scheduled payment due date.
# There is no annual percentage rate (APR) or finance charge associated with the payment Plan.
# First quarter (initial) students can submit this form starting with their second quarter.
